12)  German Wirehaired Pointer Rescue, Inc. will not be held liable for any loss or damage to property, or be held liable for temperament of a shelter animal. It must be acknowledged that all dogs must be kept separate for a minimum of 10 days from other animals and crate trained. Dogs will be given daily walks, leashed trained and exercised. Signing this agreement will also insure that the foster home understands that all medical care must be approved, and GWP Rescue 913-602-9124 will be notified and allowed to speak to the vetting facility before treatment is given.  All rescue dogs are property of German Wirehaired Pointer Rescue, Inc, and while the foster home has input on where the dog/puppy goes…the final decision is that of the rescue group as a whole.

FOSTER CARE AGREEMENT

  I agree to the following conditions: (Please initial each)

 

1. ______ I certify that my own pets are currently licensed and up to date on

                 his/her vaccinations, including rabies.

 

2. ______ I agree to keep my pets separated from the foster animal for at least 10

                 days. If the foster animal is incubating any diseases this separation will

                 minimize the chance of my pets becoming ill.

 

3. ______ I agree to keep the foster animal indoors unless accompanied outside

    

 

4. ______ Should the foster animal become ill while in my care,  I agree to call a designated vet approved by GWP Rescue, inc. All charges must be reviewed beforehand as dogs care is capped at $300.00 for shots, microchip, spay/neuter, hw test.   

 

5. ______ I fully understand that the foster animal is the property of GWP Rescue,Inc.

7. ______ I understand that the GWP Rescue, Inc is not

                 responsible for any property damage and/or injuries that may occur.

                 Any damages and/or injuries will be my responsibility.
